Tirzepatide 101: How to Reconstitute
Collapsible content
What kind of water should I use to reconstitute?
Always use the bacteriostatic water that is included on your kit to reconstitute tirzepatide. BAC water contains 0.9% benzyl alcohol, which acts as a preservative to help prevent bacterial growth. This allows you to safely store the vial for multiple uses over several weeks.
Do not use sterile water — it does not contain a preservative and is only meant for single-use mixing. Using sterile water may increase the risk of contamination or infection if the vial is punctured more than once.
How much bac water should I use to reconstitute?
For simplicity, we recommend:
15MG vial → 1.5 ml bacteriostatic water
30MG vial → 3 ml bacteriostatic water
How do I draw my bac water from the vial?
Before use, clean the vial’s rubber top with an alcohol swab. Use a new, sterile 3 ml syringe.
Start by pulling back the plunger to fill the syringe with air equal to your intended amount. Then turn the vial upside down and slowly and accurately draw 1.5 ml of bac water.
TIP: Keep the needle tip below the liquid line to avoid bubbles. If bubbles appear, tap the syringe gently and adjust before injecting it on your tirzepatide vial.
How do I inject bac water into the tirzepatide vial?
Clean your tirzepatide vial top with an alcohol swab.
After cleaning the vial, using the 3ml syringe you used to get 1.5ml of bac water, insert the needle at an angle, touching the inside wall. This prevents spraying the powder directly.
Let the bac water enter slowly—do not force the plunger the vacuum will gently pull the water in, this prevents splashing and helps the powder dissolve evenly. Peptides are fragile and can be damaged by pressure.
Do I need to reconstitute before each use?
No, you only need to reconstitute once per vial. After mixing tirzepatide with bac water, store the vial in the refrigerator between 2–8°C (36–46°F) do not freeze.
The reconstituted solution stays stable for up to 28 days. Do not freeze or expose it to heat or direct sunlight. Use a clean, sterile syringe each time you draw your dose to avoid contamination.

Administration and Dosing
Tirzepatide is injected subcutaneously, meaning into the fatty layer just under the skin. The most common injection sites are the abdomen, outer thigh, or back of the upper arm.
Use a new, sterile insulin syringe each time. Pinch the skin to create a fold, then insert the needle at a 45–90° angle, depending on the area and fat thickness.
Inject slowly and steadily, then withdraw the needle and dispose of it safely in a sharps container. Rotate injection sites each week to avoid irritation or bruising.
Tirzepatide is administered subcutaneously once a week, typically starting at 2.5 mg and increasing up to a maximum of 15 mg depending on individual needs and tolerance.
Subcutaneous injections are given into the fatty tissue of areas such as:
- Abdomen
- Thighs
- Upper arms
An initial dosing schedule helps establish your titration protocol and guides you toward your maintenance dosage. Many medical providers follow a stepwise approach, such as:

What is Titration and Why Do We Use It?
Titration is the process of gradually adjusting a medication dose based on how your body responds. It starts with a low dose and increases over time to reach the desired therapeutic effect while minimizing side effects.
This method is used in many treatments such as diabetes, high blood pressure, and psychiatric care because finding the right dose is essential for both safety and effectiveness.
For tirzepatide, dose titration improves tolerability by giving your body time to adjust.
- Start low: Allows your healthcare provider to assess your response.
- Increase slowly: Reduces the risk of side effects.
- Typical schedule: Increase the dose every 4 weeks until you find your optimal dosage which is often between 5 mg and 15 mg weekly.
- No need to increase if stable: If you are responding well and not experiencing side effects, you do not need to move to a higher dose.

FREQUENTLY ASKED QUESTIONS
How to properly store peptides?

While reconstituted peptides can last up to 12 weeks in the fridge, it is recommended to ONLY use within 28 days for maximum potency. It can be used up to 35 days only with caution, ensuring proper refrigeration and minimal exposure to temperature fluctuations.
What’s the best time to inject tirzepatide?
There isn’t a “best” time to administer your dose, it depends on what’s most convenient for you.
Here are some tips:
- Meal timing: Ideally, inject 2–3 hours before or after your first meal to optimize absorption and effectiveness.
- Choose a low-activity time: Pick a day and time when you have minimal activities, in case you notice mild side effects like dizziness or nausea. This helps ensure your routine isn’t disrupted.
- Morning or evening? Some people prefer morning injections to start the day fresh, and if you want to monitor side effects while you’re active during the day.
While others feel evening injections fit better with their schedule. Ideal if you prefer to sleep through any mild side effects, making it less disruptive to your routine.
Either works as long as it’s consistent.
- Consistency matters: Whatever time you choose, try to stick to the same window each injection to maintain steady results.
Remember, the goal is flexibility and comfort—the best time is the time that fits your lifestyle while keeping your routine simple and safe.
When should I increase my dose?
Tirzepatide dosing is gradual and individualized. Here’s what to know:
- Follow the prescribed titration schedule: Usually, doses start low and are increased every 4 weeks, depending on tolerance and response.
- Do not increase yet if your current dose is working well for you and side effects are minimal.
- Monitor how you feel: If you’re tolerating your current dose well and your provider recommends, you may move to the next dose.
- Never increase on your own: Skipping steps or increasing too quickly can cause side effects.
- Track progress: Keep note of weight changes, appetite, and any side effects to discuss with your provider before increasing the dose.
Tip: Patience is key—tirzepatide works best when titrated gradually, giving your body time to adjust.
What if I miss a dose?
Take it as soon as possible if it’s within the same day. If it’s almost time for your next scheduled dose, skip the missed one and continue your regular schedule.
Never double the dose to make up for a missed injection.
What if I don’t experience side effects or weight loss during my first weeks with tirzepatide?
It’s completely normal not to feel side effects or see immediate weight loss during the initial dose or first few weeks. Everyone’s body adjusts at a different pace.
- Be patient: Tirzepatide works gradually, and results often appear after several weeks or months.
- Monitor your routine: Make sure your diet, activity, and health habits support your goals.
- Stay consistent: Continue following your prescribed schedule and dosing.
Tip: Lack of side effects early on does not mean the medication isn’t working—it often indicates your body is tolerating it well.
